Clinical Experience and New Developments

Single 10 mm graft after 3 years
(Vladimir Bobic, 1996)

Though transplantation of autogenic (from yourself) osteochondral grafts has a history dating back to the late 1970's, the specific technique called "OATS" or "mosaicplasty"was first reported in the early 1990's. The results have been quite favorable in terms of pain relief and the ability to transplant and maintain the hylane type cartilage. In 1996 and 1997, Hangody et al. published the animal trials and their extensive clinical experience (over 370 procedures) demonstrating the medium term (longest-6 years) survival of the hylane articular cartilage. As for new developments, larger harvesters (10-14mm) have been developed to fill larger defects with one harvest. Filling donor defects with hydroxyapatite (HA) rods, bioactive growth factors to enhance integration of the hylane cartilage and biological glues to seal the cartilage interfaces have all been on the forefront of the OATS development.
